Tell us about the trip — we’ll handle the rest.
Send us the details below and a coordinator follows up with a quote for your specific trip before anything is booked. For a same-day or urgent transport, calling is fastest.
How are you booking today?
This sets the flow once. Everything after is the same — a facility or case manager just gets a few extra fields at the end.
Five simple steps from your first call to a safe arrival.
Every trip follows the same clear process, so you always know what to expect.
Tell Us About the Trip
Call or send the request below. Tell us about mobility, equipment like oxygen, and the appointment so we arrive prepared.
We Confirm and Reserve
We match the right vehicle and Specialist, take a card on file with a pre-authorization, and confirm by text and email.
Door-Through-Door Pickup
Your uniformed Specialist comes inside to get you and secures the wheelchair before the van moves. Please be seated in the wheelchair at pickup; we can’t transfer from a bed or another chair.
Cared for, and Kept Informed
A calm, safe ride. You and your family get text updates and a live tracking link.
Safe Arrival and Return
We bring the client inside to check-in. For the ride home, call us when you’re done, or add Wait & Return (paid add-on).
Booking questions we hear most
Still unsure? Call (847) 585-8888 and we’ll walk you through it.
Yes — most reservations are made by a family member, a caregiver, or a facility on a client’s behalf. Choose “for a facility or case manager” at the top of the form if you’re booking on an account; otherwise everything stays in one simple flow.
Every trip is quoted individually — there’s no published rate card, because the cost depends on the route, the mobility and equipment needs, and the timing. Tell us the details and a coordinator follows up with an itemized quote before anything is booked.
Your request reaches our reservations team at reservation@rainbowroadllc.com. A coordinator reviews it and gets back to you to confirm the details and go over the quote. If the trip is same-day or urgent, calling (847) 585-8888 is faster than the form.
No payment is taken on this form.
When we confirm the reservation, we take a card on file with a pre-authorization; the balance is due on the day of service.
Other payment options are available through our portal or a payment link, and we can mail a printed receipt if you’d rather not use email.
Pick whichever is closest. A Transportation Specialist confirms every detail with you before dispatch, and we’ll adjust the vehicle and the level of assistance if anything changes.
For round trips the return is normally will-call — you call us when the appointment is finished and we come back.
At peak times a will-call return can take longer, and we do our best to arrive as early as possible.
If you’d rather not wait, ask about Wait & Return, an optional paid add-on where your Specialist stays nearby.
